a) Please can you provide the total number of waiting list removals, removed using or as a result of using these apps, per app in the last 12 months.

b) Please can you provide a breakdown of total removals related to each app by Index of Multiple Deprivation quintile? Please can these be presented as counts and percentages (e.g. 20% of removals fell into IMD1).

c) Please can you give a breakdown of total removals related to each app by age group. Suggested age brackets are <18, 18-25, 26-34, 35-49, 50-65, >65,- Please can these figures be presented as counts and percentages.

d) Please can you give a break down of total removals related to each app by sex. Please can these figures be presented as counts and percentages.

e) In regards to the patients who were removed from waiting lists referenced in part a), how many were re-referred to the waiting list they were removed from within one month? Please can these figures be presented as counts and percentages.

This is a request for information under the Freedom of Information Act 2000 regarding the local rollout and implementation of the Federated Data Platform (FDP). Please could you provide full answers to the following two questions and supply copies of the documents requested where appropriate.

In the below questions the term live, in relation to the FDP, refers to the trust having the platform available for use, known as a local instance. If the trust has a live local instance, but has not adopted or trialled any FDP apps, please only answer question 2.

1. a) If the FDP is currently live at your trust, please can you describe the process/methodology by which the trust has internally monitored the roll out of individual apps in terms of effectiveness, potential risks, patient impact/engagement, appropriate data management and cost effectiveness. Please could you specify the types of assessment tools used, and provide copies of them including, but not limited to copies of the following documents:

– Data protection impact assessments.
– Equality impact assessments.
– Information governance committee assessments/reports.
– Risk assessments.
– Business case.
– Board minutes discussing the Federated Data Platform.
– Implementation plan.
– Academic research or evaluations

b) Please could you also list any working groups or committees which had oversight of, or responsibility for, the roll out and evaluation of FDP apps and what their remits were.

2. If your trust is live (has an active local instance of the FDP), but has not deployed any Apps, please can you:

I) Explain why no apps have been deployed.
ii) Are you currently looking at trialling or adopting any FDP apps.

1. We are a law firm and act for Health Workers 4 Palestine (“HW4P”), a private company limited by guarantee (Company number: 15544250). HW4P advocates for the rights of Palestinian health workers, and its aims include ensuring that healthcare workers are protected and supported globally during humanitarian crises and armed conflict.

2. The purpose of this letter is to submit a FOIA request on HW4P’s behalf, expeditious compliance with which is necessary for its vital work. The request is set out at paragraphs 4-5 below.

What criteria are used to triage (Accept or Reject) referrals from the community, for patients with suspected prostate cancer, including Prostate Specific Antigen (PSA) thresholds and Digital Rectal Examination (DRE) for asymptomatic patients? Please include specific PSA levels used as well as any other criteria

I would like to request from SEDS and The Sussex Family Eating Disorder Service, for each of the calendar years 2022, 2023, 2024 and 2025 (to date, if full-year data is not yet available):

I hope you are well. Between 20 May and 30 June 2025, the Equality and Human Rights Commission (EHRC) ran a consultation on proposed changes to their Code of practice for services, public functions and associations (the consultation).

I would like to please request a copy of your trust’s response to this consultation. I am aware some bodies submitted responses to the consultation via the EHRC’s web form, while others submitted documents formatted themselves.

FDP Rollout

2000 regarding the local rolllout and implementation of the Federated Data Platform (FDP), provided by Palantir Technologies.

Please can you provide me with the following information:

1) Is the Trust already using the FDP? If the Trust isn’t already using the FDP can you please provide me with a specific rollout date?

2) It has been widely reported that there have been hidden and unbudgeted for costs to implementing the FDP at local and national levels. What is the projected and locally budgeted for cost of the rollout and implementation of the FDP that hasn’t already been provided for by NHSE national funding from the initial FDP contract? This might include, but is not limited to, extra training costs, staff costs, consultancy workers, hardware, software, licenses, or other IT related costs.

4) What new jobs or roles have been created within the Trust to enable the implementation, development and running of the FDP? What other internally allocated resources in relation to the rollout and implementation of the FDP are there?
