Response published: 2 October 2026

FOI Request

Dear Sir/Madam, I am writing to request information under the Freedom of Information Act 2000. I would be grateful if you could acknowledge receipt of this request and look forward to your full response within 20 working days, as stipulated by the Act. 1. Caseload and Antipsychotic Prescribing 1.1 Total number of adult service users on the caseload for secondary mental health services in each of 2023/24, 2024/25 and 2025/26. 1.2 Of these, the number of service users prescribed oral and/or depot antipsychotic medication in each year. 2. Physical Health Monitoring 2.1 Percentage of antipsychotic-prescribed adults with a completed annual physical health check recorded in each of 2023/24, 2024/25 and 2025/26. 2.2 For each check, the percentage completion rate for each component: HbA1c/blood glucose, lipids, liver function, blood pressure, weight/BMI, ECG (where indicated), prolactin (where indicated). 2.3 Was monitoring was undertaken by secondary care, evidenced via primary care, or via shared-care arrangement? If so, what was the number of such arrangements, and whether each is a formal, written, GP-accepted agreement consistent with NICE CG178. 2.4 Copies of current trust-wide policies, protocols, service specifications, MOUs or shared-care agreements setting out responsibility for this monitoring. 2.5 Whether an internal audit of physical health monitoring completeness/accuracy has taken place since your most recent relevant CQC finding, and its headline compliance rate. 3. Pre-Consultation Risk Assessment 3.1 Some services ask patients to fill in a short questionnaire before a medication review appointment (for example, DIALOG), so the clinician can see it before meeting the patient. Please provide a copy of any current policy stating that clinicians should review this questionnaire before the appointment takes place. 3.2 If you have checked whether this actually happened, please provide the percentage of medication review appointments where the clinician reviewed the questionnaire beforehand, for each of 2023/24, 2024/25 and 2025/26. I have attached the Excel sheet. Could you please share your response in the same format? Regards,

You asked:

1.2 Of these, the number of service users prescribed oral and/or depot antipsychotic medication in each year.

Our response:

The Trust does not hold the information requested in a way that can be reported on.  Therefore we do not hold the information requested.

You asked:

2. Physical Health Monitoring

2.1 Percentage of antipsychotic-prescribed adults with a completed annual physical health check recorded in each of 2023/24, 2024/25 and 2025/26.

You asked:

2.2 For each check, the percentage completion rate for each component: HbA1c/blood glucose, lipids, liver function, blood pressure, weight/BMI, ECG (where indicated), prolactin (where indicated).

Our response:

The Trust does not hold the information requested in a way that can be reported on.  Therefore we do not hold the information requested.

You asked:

2.3 Was monitoring undertaken by secondary care, evidenced via primary care, or via shared-care arrangement? If so, what was the number of such arrangements, and whether each is a formal, written, GP-accepted agreement consistent with NICE CG178.

Our response:

For service users on the Trust’s caseload with a serious mental illness, the Trust undertakes the six-point physical health check. For people on the serious mental illness register who are not open to Trust services, the check is undertaken by the GP. The Trust does not have formal, written, GP-accepted shared-care agreements for these checks. The Trust’s responsibilities are set out in its service specification as part of its contractual arrangements.

You asked:

2.4 Copies of current trust-wide policies, protocols, service specifications, MOUs or shared-care agreements setting out responsibility for this monitoring.

Our response:

The Trust’s current service specification setting out responsibilities for physical health checks is available at the following link: SCHEDULE 2 APHC GHC Service Specification

You asked:

2.5 Whether an internal audit of physical health monitoring completeness/accuracy has taken place since your most recent relevant CQC finding, and its headline compliance rate.

Our response:

All the physical health checks are reported monthly, click on link to access information; Q2.5

You asked:

3. Pre-Consultation Risk Assessment

3.1 Some services ask patients to fill in a short questionnaire before a medication review appointment (for example, DIALOG), so the clinician can see it before meeting the patient. Please provide a copy of any current policy stating that clinicians should review this questionnaire before the appointment takes place.

Our response:

DIALOG is integrated into the Trust’s care plans and is reviewed every six months. The requirement will be included in the new Assessment Care Management Policy, which is currently being rolled out. The Trust does not currently hold a policy stating that clinicians must review the questionnaire before the appointment takes place.

You asked:

3.2 If you have checked whether this actually happened, please provide the percentage of medication review appointments where the clinician reviewed the questionnaire beforehand, for each of 2023/24, 2024/25 and 2025/26.

Our response:

The Trust does not hold recorded information correlating appointment type with care-plan activity and is therefore unable to provide the requested percentages for 2023/24, 2024/25 and 2025/26.
