Dear Freedom of Information Team, This request forms part of a doctoral research project at the University of Portsmouth, exploring approaches to reducing wrong tooth extraction and improving patient safety within dentistry. Please could you provide the following information relating to the Oral & Maxillofacial Surgery, Oral Surgery, Hospital Dental Services or similar departments at your organisation. Part A – Clinical activity and patient safety data For each of the last three complete years, please provide: 1. The total number of procedures coded under the following OPCS-4 categories: • F09 – Surgical removal of tooth • F10 – Simple extraction of tooth 2. The number of recorded incidents relating to: • wrong tooth extraction • wrong-site dental extraction procedures • relevant Never Events involving dental extraction procedures Where possible, please provide annual totals only.
