Freedom of Information Request – Ref: FOI 225-2026
Thank you for your recent Freedom of Information request. Please find our response below.
You asked:
1. How many occupational therapy referrals for DFG major adaptations were made by the service in each financial year from 2019/20 to 2024/25?
Our response:
The Trust does not hold this information. It may be held by the relevant district, borough or city council, which administers Disabled Facilities Grants.
You asked:
2. Does the service maintain any system, database, spreadsheet or log that tracks DFG referrals from the point of OT referral through to confirmation of application receipt by the relevant district housing authority?
Our response:
No. The Trust does not maintain an end-to-end DFG referral tracking system. Individual referral documents and correspondence may be recorded in SystmOne.
You asked:
3. Does the service have any mechanism, automated or manual, for identifying referrals where no DFG application has subsequently been confirmed as received by the district housing authority, and if so, please provide any recorded procedure describing the action taken?
Our response:
Yes. The district housing authority sends the Occupational Therapist a letter confirming receipt of the referral and the means-test outcome; this is uploaded to SystmOne. The Trust holds no separate procedure for cases where confirmation is not received.
The Trust does not hold a separate recorded procedure describing the action to be taken where confirmation of receipt is not received.
You asked:
4. Does the service receive any routine data from district housing authorities or Gloucestershire County Council on the progress or outcome of DFG applications arising from its referrals? If so, please describe what is received and how often.
Our response:
You asked:
5. The number of budgeted but unfilled occupational therapy posts within the community/ICT OT service in each financial year from 2019/20 to 2024/25, together with the salary underspend generated by those vacancies, insofar as this is held.
Our response:
The Trust does not hold this information in the form requested and is not required under the Freedom of Information Act 2000 to create a new report.
You asked:
6. Provide any recorded report, analysis, briefing, or correspondence held by the Trust since 2023 concerning DFG delays, lost or stalled applications, or long waits for adaptations, including any material shared with commissioners or Gloucestershire County Council.
Our response:
The Trust holds no reports, analyses, briefings or correspondence within scope. Relevant information may be held by the appropriate district, borough or city council.
