Mental health patient homicides at Gloucestershire Health and Care NHS Foundation Trust and independent investigations
1. Please disclose since 1994, how many homicides by trust patients have occurred within a year of contact with the trust (and its predecessor bodies).
Please give a breakdown by year.
Our response:
The Trust can only provide information since 2019 when it became Gloucestershire Health and Care Foundation Trust.

Our response:
As above, since April 2019, four incidents have been managed in accordance with the Serious Incident Framework (SIF), and subsequently Patient Safety Incident Response Framework (PSIRF) for mental health homicides, including national reporting systems, local and specialist (where applicable) commissioners and NHSE.

3. How many of the patient homicides reported to NHS England were independently investigated under HSG 94-27?
If not all patient homicides were reported to NHS England and/or its predecessor bodies, can the trust explain why this was so?
