Please could you provide me with the below information for all Lloyds OPD within your trust’s control during January 2021 to December 2024.
- Contractor name
- Contractor Address
- Contractor code / GPHC number (which ever is available)
- Drug name
- Drug strength
- Items of each of the above dispensed
- Quantity of each of the above dispensed
- By month
- Date range January 2021 to December 2024
I would just like to clarify I would like the data to reflect the dispensing month.
The drugs I would like to see specifically are categorised as
- Special
- Special Obtain
- Unlicensed medications
I understand that the data held within the trust may present in a different format to my request. If my requested categorisation (last 3 bullet points) is unavailable, then I am requesting all dispensing data (first 9 bullet points of my request) for each of the Lloyds OPD sites from January 2021 to December 2024.

The Trust does not have any Lloyds OPD sites.
