Freedom of Information Request – Ref: FOI 049-2026

Thank you for your recent Freedom of Information request. Please find our response below.

You asked:

1. Since February 2025 how many people has your trust discharged due to missed appointments?

Our response:

2,837

You asked:

2. Please can you also provide your trusts latest action plan for how your trust treats and engages with mental health patients? Please can you state the date this action plan was launched.

Our response:

This depends on the service. In secondary care service with users with psychosis DNA is not a reason for discharge (in line with NHSE Intensive and Assertive guidance). Other service users discharge due to DNA is a MDT decision prior to discharge.

There is specific guidance for DNA for assessment in our policy “Did Not Attend (DNA), Was Not Brought (WNB) and Non-Access Policy (CLP160)”, link below

17.1 – CLP160 Did Not Attend DNA Was Not Brought WNB and Non Access Policy V2

You asked:

3. Please can you set out the latest plan on how your trust works with other agencies including the police? Please can you state the date this action plan was launched.

Our response:

As outlined in section 5.2 and 5.3 of the Did Not Attend (DNA), Was Not Brought (WNB) and Non-Access Policy (CLP160), link above.
