FOI Request

I would be most grateful if you would provide me, under the Freedom of Information Act, details in respect to the below. A. Please confirm the number of operational Community Diagnostic Centre (CDC)’s under the ICB. And of this number how many are Standard Large Hub and spoke B. Please list the locations of the sites along with their designation (Standard, Large or Hub and Spoke) C. Standard model –  which services are being provided and annual activity levels e.g. pathology – phlebotomy x amount of tests. Imaging Pathology Physiological science D. Large model – which services are being provided and annual activity levels e.g. pathology – phlebotomy x amount of tests. Imaging Pathology Physiological science Endoscopy E. Hub and Spoke model – which services are being provided and annual activity levels e.g. pathology – phlebotomy x amount of tests. F. Is audiology or hearing services including wax removal offered at any of the sites listed above? Please list the locations where it is being offered.
