Clarification Questions:
You asked:
4a. Whether the October 2026 re-audit remains scheduled to commence during October 2026, or whether it has now been postponed.
Our response:
The re-audit has not commenced and is not expected to commence during October 2026. This was a locally initiated team quality-improvement exercise, rather than part of a formal organisational clinical audit programme. The team hopes to repeat the audit within the next 12 months, subject to operational capacity and staff availability.
You asked:
4b. If it has been postponed, please provide any existing recorded decision or record showing:
-
- when the decision to postpone was made;
- the reason for postponement; and
- any revised timetable or planned completion date
Our response:
The Trust does not hold a formal recorded decision to postpone the re-audit, including a decision date or formally recorded reason. The timing depends on the team’s operational capacity and the availability of the individual staff member undertaking the work. The current intention is to repeat the audit within the next 12 months; no more specific timetable or planned completion date is held.
You asked:
4c. If the revised position is recorded in a clinical audit programme, audit tracker, meeting minute, action plan or other governance record, please provide the relevant entry or extract.
Our response:
The Trust has not identified a relevant entry or extract in a formal clinical audit programme, audit tracker, meeting minute, action plan or other governance record. The information held is that the team hopes to repeat the audit within the next 12 months, subject to operational capacity and staff availability.
