FOI Request

Dear Freedom of Information Officer, Under the provisions of the Freedom of Information Act 2000, I am requesting anonymised, aggregated data regarding adult patient metrics held within your electronic patient record (EPR) systems and submitted to the national Mental Health Services Data Set (MHSDS). To minimize administrative burden and avoid a Section 12 (Cost Limit) exemption, this request asks strictly for structured data points that can be extracted via routine database reporting tools. Please do not manually search clinical notes. Please provide the following data for the most recently completed financial year (1st April 2025 to 31st March 2026): 1. Patient Cohort Counts The total number of unique, individual adult patients who had an active care spell recorded under the following Treatment Function Codes (TFC) or classifications: • TFC 712 (Forensic Psychiatry Service) • TFC 723 (Psychiatric Intensive Care Service) • Classification Code 205 (Adult Medium Secure Mental Health Care) • Classification Code 206 (Adult Low Secure Mental Health Care) • Patients subject to a Section 37/41 restriction order under the Mental Health Act 1983. 2. Discharges due to Non-Engagement and Non-Attendance Of the unique patient groups identified in Question 1, please state how many had a "Discharge Reason (Mental Health Care Spell)" recorded as: [1] • Code 04 (Discharged due to Non-Engagement) • Code 05 (Discharged due to Did Not Attend / Non-Attendance) Please format this as a cross-tabulated table showing the discharge code count for each corresponding patient group cohort. Data Formatting Request: Please provide the information electronically in a reusable format (such as an Excel or CSV file). If a specific count is fewer than 5 individuals, please apply standard NHS anonymisation rules (e.g., scoring it as "<5") rather than withholding the entire row or dataset. You asked:

1. Patient Cohort Counts

The total number of unique, individual adult patients who had an active care spell recorded under the following Treatment Function Codes (TFC) or classifications:

• TFC 712 (Forensic Psychiatry Service)
• TFC 723 (Psychiatric Intensive Care Service)
• Classification Code 205 (Adult Medium Secure Mental Health Care)
• Classification Code 206 (Adult Low Secure Mental Health Care)
• Patients subject to a Section 37/41 restriction order under the Mental Health Act 1983.

Our response:

FY 2025-26 Inclusions
TFC 712 (Forensic Psychiatry Service) 50 Active care spell defined as ‘had a contact with the service’ – [Includes only total attendance recorded during FY 25/26]
TFC 723 (Psychiatric Intensive Care Service) 68 Active care spell defined as ‘had an inpatient episode with the service’ [Includes patients who had a PICU inpatient active spell active at any point during FY 25/26]
Classification Code 205 (Adult Medium Secure Mental Health Care) 0 Active care spell defined as ‘had an inpatient episode with the service [GHC does not have medium secure MH unit]
Classification Code 206 (Adult Low Secure Mental Health Care) 14 Active care spell defined as ‘had an inpatient episode with the service’ [Includes patients who had a Low secure unit inpatient active spell at any point during FY 25/26]
Patients subject to a Section 37/41 restriction order under the Mental Health Act 1983 10 Active care spell defined as ‘had an inpatient episode with the service’ [Includes the patients who had an inpatient active spell and sectioned under 37/81]

Note:  Active care spell has been interpreted as any patient who was in bed at any point during FY 2025/26, regardless of when they were admitted or discharged.

2. Discharges due to Non-Engagement and Non-Attendance

Of the unique patient groups identified in Question 1, please state how many had a “Discharge Reason (Mental Health Care Spell)” recorded as: [1]

• Code 05 (Discharged due to Did Not Attend / Non-Attendance)

Please format this as a cross-tabulated table showing the discharge code count for each corresponding patient group cohort.

Our response:

The Trust can confirm that the number is fewer than five. We are unable to provide a further breakdown by cohort or discharge code as this could increase the risk of identifying individuals. This information is therefore exempt under Section 40(2) of the Freedom of Information Act 2000, as disclosure would contravene the UK GDPR data protection principles.
