I would like to make a Freedom of Information FOI. request of Gloucestershire Health and Care NHS Foundation Trust with the following questions: What IT systems. do your diabetes clinicians and specialists use to manage the interactions with and care of diabetes patients, both within hospital and community settings? What IT systems. do your diabetes clinicians and specialists use to view the full GP-owned medical records of diabetes patients? What IT systems. or other methods do your diabetes clinicians and specialists use to update diabetes patients' GP-owned medical records?

Thank you for your recent Freedom of Information request. Please find our response below:
• What IT systems. do your diabetes clinicians and specialists use to manage the interactions with and care of diabetes patients, both within hospital and community settings? I can only comment on the practice within Gloucestershire Health and Care Trust. SystmOne is used as the electronic patient record for this Trust. You would need to approach Gloucestershire Hospitals trust for information on the acute setting.
• What IT systems. do your diabetes clinicians and specialists use to view the full GP-owned medical records of diabetes patients?
Our team can only view the full GP record if the GP practice uses the same electronic patient system SystmOne.. Our clinicians also use the Joining up your Information portal JUYI. to access clinical and medication information.
• What IT systems. or other methods do your diabetes clinicians and specialists use to update diabetes patients’ GP-owned medical records?
The Clinicians cannot update the GP-owned record. A letter is written to the GP outlining care and recommendations so that this can be added to the record.
