NOTE: This policies’ format is still based on the Gloucestershire Care Services NHS Trust original and the
trust details have been adjusted to reflect the merger of GCS and 2G and creation of Gloucestershire Health
and Care NHS Foundation Trust. Staff should continue to use this Policy until it has been reviewed and
updated to reflect any changes due to the merger.

Completing the MAR chart
. Complete in ink
. Ensure all entries are clear and legible
. The MAR must be completed by a practitioner trained and competent in this task
. Add the patient details on the front page or attached a patient label
. Complete the allergy/sensitivity information and date and sign no administration
should be carried out unless the allergy status is known and recorded.
. The medication details to be completed are
 Medication name as printed on the dispensing label
 Strength
 Dose
 Frequency
 Route
 End date if defined by the course
. The practitioner completing the MAR must record their name and the date the details
were entered on to the MAR
. A duplicate label from the dispensing pharmacy if available. can be added to the top
of page 2 of MAR instead of handwriting all the details
