-To which LMNS does your trust belong to?
-Is your trust a tertiary Fetal Medicine Unit?
-Does your trust do a routine late third trimester scan for every woman in pregnancy?

Regarding growth scans in the second or third trimester:

-To calculate HC do you use HC or BPD + OFD?
-To calculate AC do you use AC or TAPD + APD?
-To plot FL centile which biometry chart do you use? (eg Chitty )
-To plot HC centile which biometry chart do you use? (eg Chitty )
-To plot AC centile which biometry chart do you use? (eg Chitty )

-What algorithm do you use to calculate Estimated Fetal Weight?
(Eg Hadlock (HC, AC, FL), Hadlock (BPD-HC-AC-FL), Hadlock (FL-AC), Warsof (BPD-AC), Persson (BPD-TAD-APAD-FL), Hansmann et al, Combs, Souka, INTERGROWTH-21ST or other)

-What centile reference chart do you use to plot the Estimated Fetal Weight?

-Does your trust use GAP-GROW?

-To measure Amniotic Fluid does your trust measure Deepest Vertical Pool or measure Amniotic Fluid Index?

-Which chart do you use to plot the amniotic fluid volume?

-When measuring Uterine Artery Doppler PI and PSV which reference centile chart do you use?
-When measuring Umbilical Artery Doppler PI and PSV which reference centile chart do you use?
-When measuring Middle Cerebral Artery Doppler PI and PSV which reference centile chart do you use?
-When measuring Ductus Venosus Doppler PI and PSV which reference centile chart do you use?

Do you use the same charts for fetal biometry, Amniotic Fluid Index and Dopplers across all hospitals in your trust and all trusts in LMNS region?

Freedom of Information Request:

1. Number of post-partum referrals to the Gloucestershire Birth Anxiety and Trauma Service (or its previous incarnation as the Maternal Mental Health Service) broken down by referral source (self or professional) and by month since Jan 2021.
Why it is important: This data will be joined with information from a separate FOI to GHFT to help to give an indication of some of the wider impact and cost of the inadequate Gloucestershire Maternity Service.

2. Provide any analyses of the impact (specifically on increased demand and additional cost) on any and all services provided by Gloucestershire Health and Care Foundation Trust caused by Gloucestershire Maternity Service since Jan 2021.
Why it is important: This will help to understand some of the impact of the provision of inadequate maternity services in Gloucestershire.

3. Provide details of any discussions with the Glos ICB (or previously the CCG) or GHFT on additional demand and cost of delivery on any services provided by GHC, believed to be linked to the ‘inadequate’ CQC rating of Gloucestershire Maternity Service since Jan 2021.
Why it is important: This will help to understand the level of awareness within the ICB and GHFT of some of the impact of the provision of inadequate maternity services in Gloucestershire.

Please could you kindly complete the spreadsheet attached to provide information on the microbiology, virology and molecular tests carried out within the Pathology Laboratory at the Trust over the previous 2 calendar years (April 2023 – March 2025).

All questions relate to microbiology and virology molecular tests and traditional microbiology testing only, please do not include details for any other disciplines unless otherwise stated.

Please complete all questions where possible – if precise data is unavailable, please provide an estimate (stating that data is an estimate plus likely error margin expressed as a percentage).
