Freedom of Information Request – Ref: FOI 263-2021
Thank you for your recent Freedom of Information request. Please find our response below.
1. Please tell me in 2019/20 and 2020/21 how many patients were discharged, and subsequently readmitted: i. less than 7 days after discharge, ii. less than 14 days after discharge, iii. Less than 28 days after discharge iv. more than 28 days after discharge. Please find our response in a separate excel spreadsheet.
2. How many patients were discharged into temporary accommodation in 2019/20 an in 2020/21? Of these, how many were seen within 7 days of discharge? 3. Of all patients in 2019/20 and 2020/21 followed up within 7 days of discharge what was the method of follow up for example phone call, in person visit, attempt at calling not necessarily answered etc. 4. To allow comparison, please also give how many patients there were in total for 2019/20 and for 2020/21, and how many in total were discharged.
