Gloucestershire Health and Care NHS Foundation Trust, I would like to make the following Freedom of Information request: As described on page 6 of the National Quality Board's National Guidance on Learning from Deaths, Trusts are required to collect specified information on deaths: https://www.england.nhs.uk/wp-content/uploads/2017/03/nqb-national-guidance-learning-from-deaths.pdf Please may I receive the following data described in that guidance, on a monthly basis from the beginning of 2018 to the most recent month available: - The total number of in-patient deaths including emergency department deaths. - The number of deaths subject to case record review - Of these deaths subjected to review, the number that were judged more likely than not to have been due to problems in care If data is not available at the beginning of 2018, please supply data from the earliest point after when data was collected.

Thank you for your recent Freedom of Information request. Please find our response below.
GHC was formed on 1.10.2019, therefore, this data is only available from this date. Also, the data is not reported monthly, rather on a quarterly cycle information re mental health deaths is held annually.. This information is, therefore presented in accordance with the data available within existing reporting systems. To provide this information by month would take more than 18 hours to compile. The Trust does not provide Emergency Department care. Information provided is up to 31 March 2022.

0 representing 0.0% of the patient deaths during the reporting period are judged more likely than not to have been due to problems in the care provided to the patient. In relation to each quarter, this consisted of:

0 representing 0.0% of the patient deaths during the reporting period are judged more likely than not to have been due to problems in the care provided to the patient. In relation to each quarter, this consisted of:
