We do not hold the records to respond to this question.
c. Have a history of using mental health services with the NHS? We record on our local incident management system incidents for patients on caseload who are known to the Trust. We do not record incidents for those people not open to the Trust, i.e. the general public. Suicide data relating to the general public is held by Public Health.
As such, any records held pertain to those with a mental health history and currently on a caseload at the time of the incident.
