I am writing today to implement a freedom of information request. I want to know the number of occasions when written patient case notes green Rio subsidiary files. have been lost or gone missing at Charlton Lane Hospital from June 2012 until present. I also need to know how many of these missing case note files were for patients who had died in the hospital.

Number of occasions when written patient case notes green Rio subsidiary files. have been lost or gone missing at Charlton Lane Hospital from June 2012 until present.

How many of these missing cases note files were for patients who had died in the hospital?
We are only able to search from 01st April 2016, as the pre-2016 2gether system has been decommissioned.
We found one incident of lost records that would apply to this FOI request at Charlton Lane Hospital from 15th September 2020. The patient involved died on 17th June 2023 but at Gloucester Royal Hospital, so part 2 of the FOI request is 0.
