24 minimum mandatory questions. Additional mandatory questions appear via
progressive disclosure for some categories of incident e.g. patient falls or
pressure ulcers and depending on some answers to previous questions.
3. On average how many patient safety forms have been completed by your staff per
month over the previous 6 month period please?
1083 per month on average over the previous 6 months 01/12/2022 – 31/05/2023.
4. What is your ratio of events associated with harm to events associated with no harm
please?
38.13% harm, 61.87% no harm over the previous 6 months 01/12/2022 –
31/05/2023.
