FOI Request

I am conducting an analysis of employment-exit questionnaire data and the methods used to capture this information in a range of NHS Organisations. It is hoped that this information will inform the design of a new exit data collection process for my own NHS Trust. To achieve this, I kindly request your help under FOI Act 2000.. I would be grateful if you could fill in the attached excel document. Please can the answers be added to the spreadsheet and the completed spreadsheet returned via email before the 8th March 2024.

i. If so, is the questionnaire completed electronically? ii. If it is electronic, is the automated exit questionnaire functionality set within ESR? iii.
