We are interested in receiving the following information for each year in the period from 2019 to 2021:
• The value of medicines wastage every year in Gloucestershire Health and Care NHS Foundation Trust in £. Unfortunately, we do not collate this information in the manner which you have requested.
We are interested in receiving the following information for each year in the period from 2019 to 2021 please include total and year-by-year breakdown.: • The number of recorded cases of medication errors and adverse drug events resulting from the medication preparation of drugs prepared from vials and ampoules within Gloucestershire Health and Care NHS Foundation Trust.
• The number of recorded cases of medication errors and adverse drug events where a prefilled syringe was used within Gloucestershire Health and Care NHS Foundation Trust. 2019 2020 2021

• Gloucestershire Health & Care NHS FT are a Community and Mental Health Trust. Your request for information relates to Intensive Care Units, Accident & Emergency departments and Operating rooms. We therefore cannot answer both parts of Q2. This part of your request needs to be directed to the Acute Hospital, can we therefore suggest that you contact Gloucestershire Hospital NHS Foundation Trust who should be able to provide this information https://www.gloshospitals.nhs.uk/about-us/our-trust/freedom-information/.. Gloucestershire Health & Care NHS FT was formed in October 2019 following the merge of 2gether NHS FT and Gloucestershire Care Service NHS Trust. As GHC was formed in Oct 2019 we can therefore only provide data from this time. Please confirm if you wish for us to only provide data from Oct 2019 onwards?
