Thank you for your recent Freedom of Information request. Please find our response below.
Has your Trust published its plans for the rollout of tech-enabled virtual wards? Does this include plans for frailty?
No – this does not include frailty
Has your Trust identified a tech provider/ supplier for the delivery of a frailty virtual ward? If so, who?
No – currently in pilot for Virtual Ward
Do you have phased targets between now May 2022. and December 2023 to ensure you reach the national ambition of 40-50 per 100,000? If so, what are they?
No
What is your Trust’s current average length of time between being deemed medically fit for discharge and discharge occurring for patients aged 65 and over?
Self-funding P1 and P3 – Total bed days 114 nCTR, average wait 10 days
Homefirst P1 – Total bed days 157 nCTR, average wait 7.8 days
Domiciliary care P1 – Total bed days 123 nCTR, average wait 15 days
Nursing/Residential Respite P3 – Total bed days 1045 nCTR, average wait 40 days
Average wait per pathway in snapshot form. This does not differentiate for only 65s only, however under 65 years old in community hospitals is less common.
What assessment, if any, have you made of the cost savings of utilising tech-enabled virtual wards for frailty?
At this time, we are focusing on testing the tech and systems that are proposed with a cohort of potential service users identified as frail.
Who is the clinical lead for your frailty virtual ward?
Helen Mee for the Virtual model of Care. There is no frailty ward in place yet
Does your Trust expect to recruit additional roles to support the delivery of tech-enabled virtual wards within the next 12 months? If so, what are they?
The project is being developed at system level and it is anticipated there will be roles that will be developed but the detailed workforce modelling has yet to be completed
How do you plan to integrate with social care and local authorities for the delivery of frailty virtual wards?
The work is being led by the ICS and will be part of a system wide response. Partners in social care, LA, citizens and voluntary sector are key partners in the project
